Both office locations will stay open during our normal business hours to do our part to keep dental emergencies out of the hospital emergency rooms.

Your User ID is emailed and for security purposes, you will be prompted to set your own password, by resetting it with your email. After logging in you will need to complete the following sections: Patient Registration, Medical History, and Pain History.

When you arrive for your appointment, we will request your digital signature for each of these forms. You will also be asked to sign our Consent for Treatment. You may review a copy of this consent form online as well.
